We recently had a serious incident where a neuro muscular blocker (muscle relaxant) syringe was inadvertently used for flushing an intravenous cannula. Will prefilled syringes reduce the chances of this happening again?
Yes. Prefilled syringes of suxamethonium are available; we understand that syringes of Rocuronium may become available soon. These will all be consistently labelled with the ISO colour red that should reduce this possibility. Flushing errors might also be caused by confusion with a poorly labelled manually prepared saline flush syringe. Commercially available prefilled syringes of saline are now widely used by ward nurses, and their use in the operating theatre could further reduce this risk. Their use has also been shown to reduce catheter-related bloodstream infection rates on ICU by half compared with manually prepared saline syringes.
